When arriving for your appointment:

On your first visit to Johns Hopkins, please report to the Pediatric Oncology Outpatient Clinic. The staff at the information desk at the main Wolfe Street entrance to the Hospital or the garage entrance  can direct you to the location of the clinic.

If your child is scheduled to be admitted as an inpatient at the Children's Center, you will then register in the main Children's Center admitting office, CMSC 131, on the first floor, near the Hope Forest Children's Lobby.

For follow-up visits, you should check in at the Pediatric Oncology Outpatient Clinic desk.

You will need to bring the following items with you to your first appointment:

  • Health insurance card or policy number
  • Insurance claim forms (available through your insurance company or employer)
  • X-rays, pathology slides, and other test results from your referring physician (if they have not been sent in advance)
  • Your child's medical records
  • Record of your child's immunizations
